• Certified Nurse Practitioner (NP)
• Minimum 5 years experience, preferably in long-term care
• Monday - Friday, with some night and weekend call
• Evaluate and treat patients during acute illness, make nursing home visits, discharge planning
• Active IN license, DEA, CSR, BLS
